crohns and working

I have had crohns for many years for the past 6 months I am struggling to work and am considering giving it away due to continue illness which includes chronic bone pain, extreme fatigue and diarrhea with bleeding, I take many medications which are causing side effects and I feel I can no longer work but I am worried about the financial side of this.
